Żeby ktoś się ewentualnie nie naciął: Ze stron oficjalnych, zarówno PL jak i US, a także z mapki udostępnianej np. pod skyscanner.pl , wynika że do wjazdu do Stanów potrzebny jest (po wpisie Megalomana można dodać: dla nie-obywatela USA) certyfikat. Te źródła nie wspominają o teście. Hiszpan2007 też napisał że sprawdzali certyfikat (chociaż niby mogli i test).
Co ciekawe, powyższe strony int. piszą "można wjechać ale", i tu następuje wielo-linijkowe rozwinięcie owego "ale".
Nie wiem kto odprawia w check-inie np. LO (czy inne linie tzn. którymi można polecieć z przesiadką) - może trzeba by dzwonić do handlingowców właśnie, albo do lotniskowych wizowców (jeśli to drugie można uczynić z zewnątrz)? Bo oni konkretnie będą najbardziej (!) na bieżąco.

Z tego wynika, że musisz być zaszczepiony i mieć certyfikat szczepienia przy sobie oraz musisz wypełnić attestation form.
Osobiście polecam wydrukować 1-2 kopie i mieć pod reką pdf w telefonie.
